Case Managers Case Studies

 Case Study 1

One of our clients is currently off sick from work with stress and anxiety. A Case Manager is supporting her with several issues and has signposted her to more than one intervention to date. Her stress was largely related to problems with large debts on credit cards and catalogues. The Fit for Work Service was able to signpost her to the Citizens Advice Bureau, which helped her to apply for support through the Debt Relief Order scheme.  This has helped her to bring her debt under control and has reduced the high fees she was being charged by a debt company. As a result, her stress levels have reduced, and she has been able to focus her attention on returning to work.

 Case Study 2

A client came to the Fit for Work Service experiencing an acute episode of a chronic back condition. He was taking strong pain killers daily in work. Following referral to physiotherapy, he received three sessions and was given further exercises to do at home.  With additional support from a chronic condition programme to help him develop coping strategies to deal with the pain, the client feels happier and plans to keep on working.

 Case Study 3

One client was referred to the service by her GP, having gone off on sick leave with depression as a result of issues at her workplace. Fit for Work Service staff helped her access counselling support, and also supported her to work through some of the other issues regarding her employment.  With help from Careers Wales and ongoing support from Fit for Work, the client was successful in gaining alternative employment, and started work shortly afterwards.

Employers Case Studies

Mrs X
Mrs X was suffering from impaired mobility due to a recent knee problem, which in turn was causing problems with her attendance at work. Workplace adaptations and a phased return were implemented in the workplace, however the company’s Occupational Health Nurse felt a physiotherapy assessment through the Fit for Work Service would help to support Mrs X. Fit for Work were able to arrange physiotherapy treatment for Mrs X , and over a period of weeks her mobility improved, allowing Mrs X to return to work.

Mrs Y
Mrs Y has suffered from anxiety and depression for a number of years. She struggled in the workplace due to low self esteem and lack of confidence. I referred Mrs Y to the Fit for Work service and she was able to access CBT sessions which she found very beneficial in coping with her anxiety and depression.

 Mrs A
This lady could not attend work following serious surgery to her foot some months ago. The company was able to provide seated work, but as she needed two buses to get here, she could not take up the offer. The Fit for Work Service arranged for short term transport through Access to Work and she came back earlier than anticipated.

Mrs B
Mrs B was off work for some time prior to a recent bereavement. Following her loss she was very low and struggled to return to work as she had lost confidence and was very weepy.
I referred her to the Fit for Work Service and she was able to access counselling quickly over a period of weeks. Before the counselling was complete, she was able to return to work and went through a phased return to normal hours/duties as she felt better.  She indicated the counselling had been an important step in her recovery.
